The website says:
" Please use the following consent form when submitting an ATIP request: Consent for an Access to Information and Personal Information Request – IMM 5744."
You don't have to worry about this form. It's only needed if you are applying for CAIPS on somebody else's behalf. Just fill out the form (Privacy Act), then print it out, sign and scan it. Scan your study permit and driver's license, then apply through email.

It's the history that was the problem. He had enough money, but the immigration officer was not convinced that the money was his.

Every student here (in my university) used to do just RA (taxable), but starting 4 or 5 years ago, they switched to GRA (or GRS as we call it here in Waterloo), which is non-taxable. A student is an RA when he's working on some research project that is not related to his PhD thesis, whereas GRS is the funding given to students to work on their own PhD projects. As far as CIC is concerned, GRA/RA/GRS should all be the same, as students perform the tasks and main duties as given by NOC 4012.
