What is strange in my case is that my bank told me the DD was encashed a lot later, on 25th of July.

Following is the reply i received today:
Good Afternoon:
New Photos were received and attached to your file.
Please be advised that your application was returned with all attachments on 3 July 2014. It was deemed incomplete. A letter of explanation was provided along with the application package.
We do not keep detailed records of each application returned so I am unable to provide more case specific details.
Please note: all returned applications are sent via regular mail to the mailing address as entered on the application package. There is no tracking number. Once the application is mailed, its delivery is the responsibility of the postal service. Applications are not sent via courier.

I am preparing another application right now & don't want the same mistakes to occur this time.
The problem area according to me in the first application were:
1. Birth certificate without my name (It had required copies, translation & affidavit)
2. Reference letter with only 20-30% matching with NOC description.
Which of the above points would have caused my application to be deemed incomplete.
To address the above points, i have done:

This time i have made a refrence letter on plain a4 sheet signed by my supervisor and also made a copy of it on a affidavit which is again signed by my supervisor, with his visiting card, copy of his company id card, affidavit explaining absence of letterhead, Company issued service letter on letterhead without all positions & all duties, promotion letters, payslips & income tax certs..Century said:Sorry to hear this brother.
Any of the two is good enough to be concluded as incomplete. Nithyaindia, 12th May applicant, received his package. You may consider waiting one more week if there is other reason.
Add a cover letter why Letter Head cannot be attained.
