do i need to provide T4 or NOA with my application

No need for the T4 or NOA when sending your application. Just check of the box authorizing IRCC to contact CRA. I did bring the NOA to my test/interview tho just to be sure but they were not required.

What success2016 meant with local office was your processing office in Canada. This would be the address on your test invite and this is where you would send request for rescheduling/explanation for missing first test.
