Does it say on the email from CIC or VO? normally you should send requested docs or medical in a month after you if you cannot make a time, email to your VO with explanation and ask more time, or at least let them know it would take some time.

don't need to put short trip (less than 6 month ) on the form. one of question ask when is recent entry to Canada, she can put May 10th, 2016 with port of entry.
