Questions on where to send requested documents

CIC requested we send them form IMM5409, Statutory Declaration of Common-Law. All their letter states is that we must send the signed and completed form bearing original signatures and the deadline for the form.

Do I send it to the same place I sent in our original application? Or is there another address?

I was told to send to ottawa office then they would send to regional office.

so I would call CIC to ask.
