Get a notarized affidavit or document and submit it. As long as the document is notarized it's valid. I did the same for my 8 year old son and it's been considered by CIC.

As long as I know ( personally learned over call to CIC) You can't have 2 Mycic account as once created it is connected to your UCI . Which means you can get Haley's and reset account but can't have 2 Mycic. Not sure about OINP and 2ND app.

All I wanted to say as per Job Bank minimum wages for NOC 6211 is 18.75 ( if not changed). Moreover, as per CIC website there is no Minimum wages Requirement. So, they can not deny your application on basis of wages. I hope I am making myself clear here.
