yes i think its the IMM5669E form. You can just download the blank form as stated in the email. I think they're only asking for a specific portion of the form which is the personal history details only.

thanks a lot iam_ayen. do i still have to fill out the entire IMM 5669 form which 4 pages in all, cause i was thinking of filling out the entire form but just providing the dates they ask from me, then i'll just attach the mail that they sent me.

Yes and i suggest you fill out the entire form again including the specific details they asked from you. Just make sure all info you provide in the new form coincide with the original form you submitted. Other than the specific dates they're asking and the need to update other information such as the date signed everything should be consistent. And in case you run out of space in the personal details section, you can use a separate sheet copying the same format of the personal detail section.
I think its more safer to submit a completely filled out form.

2) May I need to fill this "Additional Dependants/Declaration (IMM 0008DEP)" as all complete information already present in "Generic Application Form for Canada (IMM 0008)" so please suggest?

hi lois05, my ECAS is just the same as it was before right after i got my AOR which stated "application received". it didn't say anything about medical result received. i dunno, i think ECAS is not really that updated....

2) May I need to fill this "Additional Dependants/Declaration (IMM 0008DEP)" as all complete information already present in "Generic Application Form for Canada (IMM 0008)" so please suggest? you will only use IMM0008DEP form if the principal applicant have more than 5 dependents. I think you don't need that anymore.

In my case it took 10 days for the physician to complete our medical report since we were 4 (no additional tests taken though). it may depends how many are you and if there are no followup/additional tests. When did you have your ME @ IOM?

No news from LVO applicants after the MR of Muhammadfahad. No MR during last week, as from timeline of AOR we all are having a difference of only 3-4 days between each other. So let's hope some MR's during ongoing week.

I seriously doubt we'll have any activity from LVO soon... It's all quite in pre-November forums as well...

My guess is, it'll be atleast a month before November applicants of LVO start to hear anything... (Man.. I wish so much to be wrong here !!! )

zizu I think you are also Fsw Backlog applicant, just like fahad I know another applicant who also received MR after 40 days from LVO. So pray and hope for the best.

My ECAS status is still "Application Received". I have finished my medicals on Jan 25th, but I still don't see any update on my ECAS.

Did you send your personal history details to CIO/VO? I have updated track sheet. Hey, please take a look at the track sheet and let me know your outstanding details....there are a couple field empty in your area.

I think IMM 5669 is the right form that you and your husband have to fill in and send it to VO. If your personal history is too leangthy, just make a word document just yourself and attach it to the IMM 5669, it should be fine.

Hey ninonhet, you are mistaken....the attachment means, there may not be a physical document attachment....but they provided you with four links where you have to go and download the exact form and get it done...just choose the correct form whichever suits for your case.

Please let me know if you haven't done this yet.

By the way, what is CEM?

I think it should be fine even if you got rid of your home phone number. There is still your cell phone number in their records and your email addres as well which is quite sufficient.
