i need some help here. CEM ask for additional documents from me and my husband which is the personal history. they didn't attached any form on the email. i am assuming it is the IMM 5669 form. please correct me if i'm wrong...

Hi Emperor.....was it mentioned on your email...... because it was not mentioned on my email and even when i called cic office they said if its not specified in my email, then there is no time limitations for me but its normally better to do that in time.

Yes it mentioned in the email I received asking me to proceed for my medical and the request for RPRF.

Are you saying you have a third party and want to get rid off?
Well in my case, the CIO sent my aor and mr on january 9 and 12 to my representative email add, but when i asked my HR i got nothing from him! So i asked CIO if they can send a copy of my AOR and MR, but got the reply from email together with just the copy of my AOR, and no MR, they said that the copy of my MR will be sent to my HR since he's my third party. I replied them that my HR is not getting any communication from CIO, and decided to fill up the form IMM5476 to cancel my third party so all the correspondence from CIO will be sent direct to my email. So after almost two months, i finally got the copy of my MR! And am expecting i will receive all the communication from CPP-O from now on. It will take some times, and more patience.

thank you for the reply... I don't have third party but i do have representative whom i wish to cancel. how did you communicate with CIO initially? what process did you do?

Hi dianerose,

Your representative is consider your third party for your application. If your paper is still in CIO, you can email them right away and tell them that you need the copy of your AOR /MR. Then, filled up the form IMM 5476 to cancel your representative and send it to CIO. However, if your papers is on your requested VO already, you have sent it from there.

Is it possible to send it to CIO and then will they forward it to VO. But I'm worried about our MR because as i have browse on those posted here...others have 30 days expiration on their MR. If i'm to count back we only have 2 weeks left to finish the MR.

Hi,

Its okay, no worries. You can call the call center of CIC just to confirm if your AOR/MR is already sent to your representative email.
You can also check with them if where is your paper at this time. Because if your paper is in your VO now, you have to communicate with them. But you can request for you copy of AOR/MR to CIO.

Hi dianerose,

About your concern for MR expiration, i just noticed some of the groups here have expiration on their MR, but those groups are applicants outside Canada. I have the same worries before about the MR expiration, but if you can get a copy of your MR, that should be good enough. When i received mine, it does not showing any time limit to response from it.

Hi iam_ayen, you did your med exam at iom 2/20/2013..do you have any update? Was it forwarded to CEM? I had my med exam at IOM too last 02/26/2013. Are they fast in processing results?

But we applied outside canada, VO is in Manila. Can I call CIC even if the principal applicant is my husband? I am here in canada as temporary worker while my husband is in the Philippines.

yes i think its the IMM5669E form. You can just download the blank form as stated in the email. I think they're only asking for a specific portion of the form which is the personal history details only.

There is no specific clinic.. You are fre to choose from the authorized clinics..
