is anyone po ba na gumamit ng maiden name sa mga forms and passport, nbi at police during application then na approved visa?? im so worried right now, kasi maiden name pa passport ko. if mag change name ako matatagalan ako kasi yung sked availablity sa dfa ay january na huhhuhu..

can I use my Maiden name in my forms since my passport still in maiden name and it will take long for the appointment in DFA.
what about status? it should be MARRIED even you use maiden name instead of your husband's name?
police clearance, NBI and medical? would it be okay to use maiden name too but MARRIED status?

my passport will expire on april2018.

1. is it required to renew passport before VO request the passport?
kasi now palang po ako mag apply. so by next year 2017 on going process n sya. mga months nlng ang magiging validity ng passport ko pag nagkataon.
and
2.since maiden name gamit ko sa passport. once na magrenew ako passport still maiden name p din gamitin ko right? kasi un gnamait ko sa application.
3. CFO- required din ba un pag nagpunta ka sa canada? if im not mistaken cfo certficate ay ginagamit sa pagchange name sa passport right?since maiden name gamit ko sa application okaylng ba na hindi muna ako umatend seminar sa cfo?

Thank you po. Malapit na cguro ako dahil sa notes na nirequest ko ay eligibility, medical, at criminality passed na si hubby.. kaya security nalang :)

I'm sorry to say, marshie, once you get married you have to change your status. Yun ang tinitingnan ng Canadian immigration na if there's any change sa status mo, you have to let them know. I have to go through all the miserable red tape just to update everything before we submitted our application. But you know what, it is actually faster kapag walang problema when it comes to processing my paper. Six months tapos na. Cheers!!!

Hi marshie,

When you fill out the forms, use the same name as shown on your passport. But of course, your status should be Married. Having maiden shown on your passport is no big deal. I got married early this year, renewed my passport in June 2016 and I still used my maiden name with my new passport. My status though, showed Married when I filled out the form. DFA took a copy of my marriage certificate and a copy of certificate from CFO.

1. Passport should be valid at least 18 months from the time you submit your App.
2. It is your choice to use Maiden name or Married name when you renew your passport. If you will choose to use Married name and your app is still in process, you will have to notify CIC via CSE.
3. If you go to CFO to attend GCP and get a certificate (yes, required when renewing passport for those married to foreign nationals), you will still have to go back to them at a later time with your Certificate once you already have your Visa and COPR.

FYI: Showing or using maiden name for females is no big deal in Canada. Status is important, op kors. Even us here in Philippines, not many may be aware of, but it is a choice if a married female wants to retain her maiden name or change to married name. Same thing, status is important.

really appreciated it! I was so worried as I am using my maiden name inthe application and was confused and wondering if I get government certificate like NBI, POLICE etc ang use maiden name but married status might cause conflict and hard to get.

speaking pf POLICE CERTIFICATE/CLEARANCE. is it NBI only or NBI and police certficate?
i worked in japan 2013 and went back to philippines 2014. Some says that CIC proabably request a police certficate from specific country if I worked abroad before.

do you have any idea about that? is the police certficate can get in japan embassy or should I request to japan. it will be another stressfull day again :(

POLICE CERTIFICATES:
If you require police certificates from Japan, Hong Kong, Singapore or Brunei, e-mail us at MANILIMMIGRATION@international.gc.ca with the subject line “Request Letter Required for <specify country> Police Certificate” in order to obtain the additional letter required for the police clearance.

NOW, im doing my appointment in DFA for renewal of my passport since may passport is now 16 months valid na lang. Would it affect my application cause we are plan to apply this december. Now, my confused AGAIN!! haist. im filling out the form online and I use maiden name still but put married. there is a SPOUSE INFO.do I need to fill out that one too right? hindi ba sya conflict dito sa sa atin? kasi alam nyo naman dito sa pinas pag kasal automatic all valid ID's and docs should be filled in married name and married status..

As @prvc said, only NBI. No need to get local police certificate prior to applying for NBI clearance. What CIC meant on Police Certificate for Philippines is NBI. When you apply for one (online na lang para mabilis, here's the link: < https://www.nbi-clearance.com/ >) make sure you put Married for your Status. I actually just came home from NBI :)

You should not cancel your CFO sched. You need the certificate from them before you can renew your passport. That's the reason I had to attend one in March because I had to renew my passport. You might want to re-read my earlier response :)

I think you should still be okay. Pagdating mo kasi dun mag fill up ka pa din ng form then saka pa lang i-input ang info details mo then they will capture a photo for the certificate. You will show them the "online" form that you filled out and sabihin mo na lang na nagkamali ka ng lagay ng name mo. I don't think it will be a big deal kasi nga bibigyan ka pa din ng form dun pagdating mo.
