Advisory on Marriage
Nbi
Passport (12 months valid from the date na mag PPR)
New pictures with new specification

You're welcome! and naku I totally understand what you mean, the day after I sent our application I had a mini-panic attack because on the checklist, it said to put an 'x' on the boxes and I forgot if I put an 'x' or a checkmark hahahaha after ilang minutes narealize ko parang naloloka na ata ako :P Ano bang pakialam nun visa officer kung x or check, basta filled up un check list haha
I just read this and your dilemma made me panic din tuloy haha. About to submit our application today and I was reading dito to make sure na complete, and true enough hindi ko din na 'x' yung boxes, check din haha. I hope everything goes well though. Thanks again for your help!

Nagemail ako s hk to follow up at ng reply sila na binigay ang reference number nya at accepted/on process palang daw. Nagantay kasi kami ng request letter sa CEM ng police clearance but now as I can see mga sinesend na aor2 s iba sabi s letter nila "if you need a request letter for police clearance email us" subject: requesting for police clearance (country).. You dont need to put in your letter if student ka sa ibang country basta umabot ka ng 6 and more months s ibang country automatic na kailangan mo kumuha. In the body of your letter input mo n lang ang file number mo para alam ng CEM input nila sa letter. Hope this help..

I finally submitted our application package today via good old Canada Post! I can't wait for my husband to be with us again. I guess ako pa lang ang June applicant, but good luck to everyone, and thanks to this forum kasi ang dami kong natutunan and nakatipid ako sa lawyer's/consultant's fees!

Congrats!

You write whatever name is in your passport that you will use to travel to Canada (so the one with your maiden name :)).

Thanks, Xabibella! I will just send two IMM5406Es, one bearing my maiden name and one with my married name since they instructed me to get NBI using my married name. I guess I'll just have to explain why i sent two copies of additional family information form. I hope it'll be okay with explanation. Thanks for the reply tho! :)

Subsection 1(3) of the Regulations defines “family member” in respect of a person as
(a) the spouse or common-law partner of the person;
(b) a dependent child of the person or of the person’s spouse or common-law partner; and
(c) a dependent child of a dependent child referred to in paragraph (b).

All family members, whether or not they are accompanying you to Canada, MUST be declared and presented for examination to assess whether they are admissible to Canada. A false declaration on your IMM5406 (Additional Family Information form), any failure to declare a family member, or failure to present a family member for examination constitutes misrepresentation and may render you inadmissible to Canada for five (5) years.
