It has 3 pages. the first one is from DFA, the second page is from the office of the president (white paper) and the last page is the live birth certificate which is printed on NSO paper.

Live birth certificate has no expiration right? Only the NBI clerance has a 3-month validity.

Yes, birth certificate has no expiration but if i were you, i better get a new copy since it is very easy to get one thru online. Keep the birth certificate with red ribbon, coz that one is much harder to secure and you might need it another time :)

Good afternoon! I just need some insights.

March 27, 2014 Application received by CIC.
Last August 5, the Canadian Embassy - Manila asked for my Passport.
On August 7 I sent them my passport.
Yesterday Nov.4 I received a request letter for Medical.
I am 10 weeks pregnant as of the moment.
I just want to ask, has any of you completed the medical tests needed while pregnant? I'm afraid to undergo X-ray procedure although chest X-ray is less harmful to my abdomen. I was told by St.Lukes Hospital that they will put my result on hold if I won't be able to complete the tests. They asked me to ask my OB first if it's allowed. I can't reach my OB as of the moment, I'm wondering if any pregnant applicants were allowed to finish all required tests?

Hello March 2014, I're read from previous thread that X-ray is usually postponed until you give birth. All test can be done and can be submitted to CEM and just inform them about the pregnancy. And im afraid this will put a hold on the processing of your application until you complete the Xray. (Although im not 100% sure about this) try to read from other thread here in Canadavisa forum.

Why didnt you had an upfront medical before you submitted your application in Mar14?
