Nov 2011 - Embassy of Canada requesting for our updated documents such as the NBI certificates, CENOMAR and my son's Certification of Selection of Quebec
April 2012 - Requested Documents Sent to Embassy of Canada except my son's Certification of Selection of Quebec because there no release yet
Nov 2012- Receive from my Mother, the Certification of Selection of Quebec of my son and the rest of the family including hers to Embassy of Canada
Feb 2013 - Certification of Selection of Quebec Sent to Embassy of Canada
October 2013 - My father was withdrawn from the application from Embassy of Canada
November 2013 - Medical Examination Request Receive

Its landmark is Pacific Star Building. Just tell the driver if you take a cab, back of Pacific Star Building...
For canadian visa applicants, these are their fees in php per person:
age 0-10yrs - 2,000
age 11-14yrs - 2,500
age 15 up/ Adults - 5,000
Tests performed per age group:
age 0-10yrs - Urinalysis and complete physical exam
age 11-14yrs - Chest x-ray, Urinalysis and complete physical exam
age 15 up/ Adults - HIV/Syphillis Testing, Chest x-ray, Urinalysis and complete physical exam
** Results out within 3hrs for both x-ray and lab tests. In case everything if okay, they forward the results in less than a month to the CEM.
Their clinic is open from Monday to Friday only, and by appointment basis, which means you need to phone them and set a schedule available for us to get an open slot from them.
They perform the medical exams per batch basis starting 7:30am then 8:30am, 10am and 11:30am.
Cut off time is 11:30- for them to give enough attention to applicants who need repeat tests and those that need to undergo additional tests...

For now they accept cash payments only, but they said their working out to have POS machines for cash cards and credit card payments..
