Since I've never been abroad and this will be the first time I'll process such documents, I would need to clarify how to proceed with some details para zero error (I hope). I'm trusting the experience of members in this forum. Your knowledge and help will be greatly appreciated! *bows*

I got married this October lang. My husband went back to Canada and we're aiming (hopefully) early next year makasunod na ako sa kanya. Provided I supply necessary requirements ASAP.

I've read Philippines [IMM 3905] document from Application to Sponsor a Member of the Family Class webpage.

Eto po mga questions ko:

1. Dito sa Pilipinas, alam ko ang babae pag nagpalit ng surname, she has the option on how her surname will come out. Either,
[ First Name + New Surname ] e.g. Joanna Marie C. Cruz
[ First Name + 'Maiden Surname - New Surname' ] e.g. Joanna Marie C. Soriano - Cruz

My question is, sa pag fill out ng forms, which format should be used? Some countries are particular with such details so gusto ko po malaman kung ano dapat gamitin sa forms para avoid hassle to filling out again.


2. According to guidelines, Police Certificates and Clearances are required. I'm assuming this entails the NBI clearance?

If so, do I have to wait for my surname to be NSO registered muna or can I get one now but using my maiden surname?

FYI - Okay na kami sa civil registrar. Hindi pa kasi umaabot sa NSO ang marriage certificate namin. Still waiting updates on it.


3. For medical exams, it states under Medical Exams page...

"If you are applying under the family class and being sponsored as a spouse, common-law partner, conjugal partner, or dependent child, then you should complete your medical examination before submitting your application."

Does this entail I could proceed with getting an exam or should I wait for instructions before I do so?

I was told from some VISA applicants (I'm not sure on under what category), they were to wait for instructions before proceeding. How would it go under spousal category?


4. I've checked on DMP's in Makati area (taga Makati po ako). I personally prefer female doctors and I found two physician's list. Susana Camille Timbol-Sanchez and Ma. Theresa Pinlac.

I would like to ask if anyone underwent medical exam with either doctors and how it went. If anyone could refer me to a different and good DMP within Makati area, I would greatly appreciate it.

Yon po questions ko as of now. I hope I could get replies from anyone soon.

1. You can put your name + maiden name + husbands surname. No need mo n ilgay ung middle name nung dalaga k p.

2. S nbi,pwde ng ilgay ung new surname mo,kc llabas pdn s nbi ung maiden name mo.

3. S medical,since spousal class tyo,mgpapamedical muna bgo isubmitt ung application nio. Kc kelangan nla ng copy 2 n gling s medical pg ipapasa nio n application nio.

4. S medical if you're a female,female physician dn mgeexam syo.so dont worry about it. So kht saan k nlng malapit if you want.

So far yan po maitutulong ko base on my experience also.first time ko lng dn s mga application n gnto pro nkaraos nman s processing.haha

Ung Old nbi po dalhin nyo sa renewal tapos ilagay nyo lang surname ng asawa nyo ..alam po nila un pag newly wed..


Sa coprehensive pulponary clinic makati babae din po doctor di sila masyado mahigpit tsaka mabait po xa..within the day makukuha mo na ung copy ng medical rport or receipt..they will call u wk after d medical kung may problem..konti lang tao don kaya wala masyado pila di gaya ng ibang clinic

Dina po ako hiningan ng marriage contract for my nbi and police clearance pero dalhin nyo na rin ung mc nyo from civ reg..

Ung sa medical pala sis they wil just ask bout ur medical history... Physical exam mabilis lang kaya di ako nahiya hehe

Name mo+Apelyido ni Papa mo+Apelyido ng Asawa tanggal na po ang middle name mo na apelyido ng mama mo . Good example nung kinasal ang mama mo sa papa mo kung paano po nya isulat ang buong name niya ngayong married na sya .

Nbi po pinasa ko dito at Police Clearance din dito po sa Pinas

Mas maganda po kung isang last name lang ang gagamitin nyo sa lahat ng Documents nyo po .

Sa Family Class Sponsorship CEM rules po kailangan bago po ipasa kay hubby sa Canada ang Mga nafill apan ng Documents sa side mo na Applicant kailangan kompleto kasama po dito ang COPY 2 from DMP Medical nyo po iyon ipapa xerox mo po at Original ang ibigay sa Sponsor sayo po xerox copy. Yung result ng medical po yung DMP na po magsesend nun sa CEM Canadian Embassy Manila. Tatawag nalang po kayo sa araw na ibibigay sa inyo ng DMP kung nasend na po nila sa CEM at walang problem pag may problem po kasi di nila isesend ang result maggagamot po muna.

Wag nyo na pong hintayin ang Instruction ng CEM para magpamedical. Kasabay po ang Original Copy 2 na ibibigay ng DMP after medical sa Application Package na isesend ng Sponsor po sa Canada.

Bago po ako pumunta tumawag po muna ako para sa mga requirements nila like Appendix C form, 7pcs passport pics white background, passport ko, di ko na po matandaan yung iba . Tawag nalang po ikaw para sure

Pagpunta ko po dun DMP SCTS-Makati may pina fill up na form ulit sa office nila, may DRA na mag iinterview sa lahat ng sinulat mo dun sa form tapos kuha ng urine test, blood test , x-ray sa room, sa office ni Dra. Susana physical exam. sa eyes, weight, may room dun dalawang Dra ang magchecheck last si Dra Susana ipapatanggal po ang damit underwear lang po matitira ichecheck kung may tattoo, piercing, tapos ang throat ng paulit ulit kakapain, breast, eyes, ears, anus then bihis po tatawagin ka ni Dra Susana sa table nya sa harap nya tatanungin ka nya ng mga questions about sa health mo po.

Kaso nag-rereview kami kanina ng hubby ko via Facetime ng ibang parts ng applications. Kaso yung iba di rin siya sure kung paano gagawin.

So I will again ask for your expertise mga friends.


1. Under ‘Schedule A Background / Declaration Form’ and ‘Sponsored Spouse / Partner Questionnaire’.

Napansin ko mga sis na may ganitong mga tanong:

“Provide details on number of attendees. Indicate if your sponsor, your sponsor’s family members / relatives and your family members / relatives attended and if not, why not. If insufficient space, use separate sheet of paper. Attach photos, if any.”

My dilemma mga friends ay… wala po ang mom ko sa moment ng ito ng aking buhay. She is not deceased.
Matagal na po separated ang mom and dad ko. Confidential po ang reasons pero ang masasabi ko lang po ay, di na talaga sila nagkakasundo at mas umayos ang buhay ng pamilya ng magkahiwalay sila.

The thing is, nawalan po kami ng communication with mama for years. So wala po sa amin ang nakakaalam kung nasaan siya ngayon. I’m getting really worried that this might affect my application. Lalo na since, under ‘Background / Declaration’ form, wala akong mailalagay na ‘Date of death, if deceased’ under mother’s details. I’m thinking this might pose questions since my mom is not with us during the engagement meeting, not in my wedding list, etc.

Am I getting paranoid that this might pose a threat for approval? My hubby said you… “You can tell them the truth na separated na sila (my parents) for years and you lost communication. Besides, it’s not your mom I am sponsoring, it’s YOU. And you don’t have any criminal records and such. You’re clean. You don’t have to worry about anything.”

Tama po ba and mindset ng aking hubby? Am I worrying for nothing? I just want to put my head (and heart) at ease. Napa-praning na ako. Hehe.


2. Question for ‘Background / Declaration Form’

Tanong ko mga friends. Sa item #1, it asks for:
Your Full name:
Family name [ input field ]
Given name(s) [ input field ]

= Sa Family Name, lagay ko yung Maiden surname ko or my Hubby’s surname?
= And then sa Given name(s), I put in my whole name minus the surname?

= Sa item #2, it asks for ‘Your full name written in your native language or script’.
= I guess kung ano ang masasagot nyo sa question ko sa item#1, yun ang masusunod dito. Ehe.


3. Regarding Passport

Napansin ko po may forms humihingi ng passport details. Napag-isip ko if need ko magpa-renew / update my passport para lumabas ang aking new surname. Kailangan pa po ba?


4. Under Generic Application Form for Canada

May nakita po akong areas asking for UCI and NID.

Ano po yung UCI?

As for National Identity Number (NID), eto po ba ay mga government IDs like SSS? Kung gagamit lang ako ng isa ano ang pinaka-magandang gamitin?

Hi guys ask ko lang sana nung nakarecieve ng approval ung sponsor, ano ung nakasulat sa letter? And ano yung PR kit na nakasulat sa ecas? D kasi nakatanggap ung sponsor ko ng approval letter e pero nung tumwag sa cic ok naman daw application nmin..and na forward na dto sa pinas nung aug.. Ok lang ba yun kahit na d narecieve yung approval letter?
