Yes u need the consent form filled and signed by the principal applicant. Sponsor needs to apply online and pay the fee and upload the consent form. Notes arrive in sponsor's email.

I am planning to apply notes. I filled imm 5744E on 19 march 2017 but not yet applied. Can i use same form or fill new one for today date?

My application was returned due to IMM0008 having my address listed as my Spouses residential address (he is back home and I am here).I sent it again and it was received April 6. My spouses medical was done on October 14, but we applied after the rules were changed. We sent the pervious medicals with our application.
