Regarding point 1: I was going through the CIC website (http://www.cic.gc.ca/english/information/medical/medexams-perm.asp) just now. As per the website: Upfront Medical Exam is only applicable to certain "Family Class Applications". I feel probably not everybody can do Upfront Medicals.
Regarding point 2: I think this surely can be done in advance.

BUT... a lot of people had issues with it. Especially with up-front medicals.
Some times they don't reach CIC on time.
Some times they struggle to 'connect' it to your application, thus it gets lost.
Some times you have very little time to land, before your visa expires.
If money is not an issue (in the case you have to do it again later), you can do Up-front medicals.
There is nothing wrong with it.
