Only scan copy. Never send any original doc to IRCC, ever.

Yes they added her for all years including previous which doesn’t sound right to me

Ok here are the steps you should take immediately:

1. Remove your spouse as co-sponsor
2. Write an explanation letter for your 16, 17 & 18 income, showing your no. of family members for that year and provide the CRA Option C printout with that
