It is an additional form requested after sponsor is approved and application is sent to local visa office. The form was requested together with police certificates and medical exam. You are not missing anything. Just follow the check list.

Hi everyone, newbie here. I'm so excited that I finally got selected in round 2 PGP and I'm preparing the application for my parents. I have a stupid question here, you may laugh at me but I really want to get the clear answer, please advise:):

My native language is not English, and both my parents don't speak English. I will fill the "Use Of A Representative" form and prepare the application for them. When I let them to sign the form, they ask can they sign it in our native language, or they need to sign it in English spelling? I checked their passports, the info page includes their full name in both native language and English, and the signatures on their passport are in native language. So does it matter if they sign in Native spelling?

Also in last page of IMM0008, it requires to declare the name for consent (in printing maybe?) and a signature, does the Consent need to be written in English only? On the form it shows "(first name, last name)" so I assume it should be in English spelling only. Am I correct? Can I let them writing the Consent declaration in English spelling and sign in native language?

Any suggestions or replies are help, thanks in advance!

Today I called cic customer care they said that they received my application and gave me temporary file no . But yet not checked by any immigration officer . I asked if I link my application to ecas they say no without original file no we can’t do that . Any suggestions

I have few more question regarding IMM 5768...

1. Question 14 - EMPLOYMENT HISTORY - SPONSOR... In this the "Gross Income for period" should be total of last 3 year's income?

2. My wife is not co-signing the application, so in Question 16 - Co-signer - Statement of Consent... What I have to select..? Yes or No? If No then do my wife need to mentioned SIN and sing that?

In addition to this, in IMM 1344... Do my wife need to sign it?

2- If she didn't sign as a co-signer IMM 1344 then you may leave this(Q-16) empty and after printing write N/A by handwriting

I think you need to be bit patient as CIC rep indicated that you will be able to see your application details on ECAS once the officer will check your complete package... The best thing that they confirmed you that they have received your application and now it's just a bit matter of time... if there's anything missing in your app then they will ask you to provide that..

I think this one is really confusing. Here in Question 3 -> B -> i -> I have to put "1" (I have sponsored my wife 1year ago and she is PR now...!!!). 3->B>i number of person included in previous undertaking you signed as a sponsor and that are still in effect( is 10 years ?) do we have to fill here wife’s 1 again as we are still responsible for it
No doubt this one is bit tricky - but you'd found where the confusion is so
will recommend leave the 3->B>i is empty and enter 1 for your spouse in Question 5... :)
Because your spouse is here in Canada and is part of your family and your dependent as well
 
Parent had colon polyps which were removed via Colonoscopy operations. However, in a recent check, doctor found a big polyps, and advise to check for any trace of cancer in next month. My questions are:


  1. If everything is fine. Do we have to mention it during immigration medical check, as there is no scar on skin resulting from Colonoscopy operations.
  2. If things are not that good and an operation with some scar on skin is needed, will Intestinal cancer make an applicant medical inadmissible? Anyone whose parent have similar situation and have passed ME?
  3. If only Chemotherapy is needed without an operation, can we not mention the colon issue to the doctor?
4. Where can I find the spreadsheet? I know the link appears in s number of posts, but it’s hard to locate it with 300 pages of posts:)

I have very dump questions but what to do I am confused! :rolleyes: and looking for advise:). I am sponsoring my parents (Father and Mother).
My father would be "Principal Applicant" right? and my mother would be sponsored as a spouse of Principal Applicant;

Q1 : do I need to sponsor my mother separately as a principal applicant and fill IMM1344 for her as well ?
Q2 : do I need to fill IMM 0008 for both mom & dad?
Q3 : do I need to fill IMM 5669 for both mom & dad? I think I should
Q4 : do I need to fill IMM 5406 for both or 1 is enough? I think I need 1
