Why dont i remember filling that for my wife? Could youplease give me the form number?
And 6 Pics? my lawyer here asked me for only 2. Are we talking about passport size pics?

Here is the form http://www.cic.gc.ca/english/pdf/kits/guides/3904E.pdf. Did you see it before?

It is from http://www.cic.gc.ca/english/information/applications/fc.asp site

But it may have changed when you applied because I cannot find it on the newer website which is more organized I found as it has forms to apply for PCC and fingerprints and Medicals which I didn't have to fill.

You can go to a photographer and show them the form paper or size that you need to take same 6 pics. Dont be confused. Its same as passport size pic.

Nothing to worry about it. Just wait for your ppr and send the 6 pics with original passport and copy of ppr letter. Good luck!
