8 IDENTITY AND CIVIL STATUS DOCUMENTS
1) For the Philippines
The following original documents are required for you (principal applicant, regardless of age) and
for each of your dependent children, if applicable:
• Birth Certificate
• Marriage Certificate
• Death Certificate
No record of marriage (CENOMAR) or Advisory on marriages
These documents must be issued by the National Statistics Office (NSO) and printed on NSO paper.
If ... Then ...
a birth, marriage or death occurred in another
country and is not registered with NSO,
provide the original certificate issued
abroad.
there are any discrepancies in your and your
family member's documents,
submit these together with a sworn affidavit
explaining those discrepancies.
your or any of your dependent's birth is late
registered or there is NO RECORD of
registration of birth with NSO,
additional original identity documents must
be submitted, such as:
• baptismal certificate,
• permanent elementary and high school
records,
• voter's ID or voter's certificate,
• old passports,
• birth certificate issued by the Local Civil
Registrar
Note: Your birth is late registered if the registration date indicated on your birth certificate is more
than a month after your birth. The registration date is the date your birth certificate was received at
the Office of Local Civil Registrar.
If you or your sponsor were previously married or in a common-law relationship, provide certified
copies of the following (as applicable), for each marriage or relationship:
• Divorce Certificate
• Annulment documents (Court Petition and Court Order/Decision)
• Separation Certificate
• Death Certificate, if widowed. Original NSO Death Certificate must be submitted if ex-spouse
died in the Philippines
• Declaration of Severance of Common-law Relationship
Has this guide been changed or updated since February 2012? I do not recall the AOM being mentioned in this guide.

And exactly how many dead people are being sponsored to live in Canada? Maybe that is what is turning this application process into a funeral march.

Oh my gosh, I didn't know you could order one and pick it up same day! What have we been waiting for! Now we are 2 weeks behind

Happy Monthisary to me, it's 11 months now since CIC received our application. Do we have any bets to how long until it's finalized? I'm betting 5 months more. Any takers? Iay, how's that rooster?

Hi Steph C, congratulations, you made it to PPR and soon a visa for your partner... This is my first time to know about conjugal sponsorship here, I've read about it, I was planning to file that before but eventually I married her to make sure sponsorship will go smoothly. Most of the people here got their visa 2 months after PPR. Good luck!

Oh yeahhhhhh.... I remember what I did now. I sent the NSO verified copy of the marriage certificate (which we waited for over half a year for), then when I saw CENOMAR I looked at all the papers that I had, found the one we used to get married, and tossed it in. So I guess we WILL be asked for an AOM.

No problem though, my wife sent me a text that she was going to get one today. So the only thing left is the wait for the dreaded re-medical. Hers expired a long time ago and our ECAS has no "Medicals received" so my guess is that by the time the app finally made it through the Ottawa maze to Manila the medical file had expired with no matching application and was tossed.

So how is St. Lukes as far as wait times?

Just a note that medical results received on ecas had been buggered since the login issues.

We're still hoping that we can have ours extended....one month before expiry (officially)...fingers crossed!

Just a note that if anyone is in the legazpi area, the NSO there has drop in pick up service for AOM's.

There is a table outside with 2 colored forms...one if your single (you want CENOMAR) and one if your married (you want AOM).

Fill out, hand in at booth1....wait for number, and go to assigned booth to pick up (5 minutes later)
The part you wait for is getting the "record" in the system...which for some reason takes a couple months even after being recorded in Manila and up to 3 months to get to Manila is sending report of marriage from over seas embassy/consulate.

Hope that helps anyone

We have been waiting over 10 days to receive the AOM in the mail. I'm beginning to wonder if we're really so ill fated as to have everything as slow as it is. When I heard from Tagum-NB that we could get it in one day.. I asked him but he complained of the 300 person hot lineup and maybe he will just wait another day in the house to see if it finally arrives ::) We agreed that if it doesn't come today he will go on Wednesday. It's like herding cats

Can your partner take Lancha or plane to an NSO office?
For some reason we weren't even told you can order by mail...that's my fear for you. We actually made 3 separate special trips becuase they couldn't give info over the phone.

1st....not in system (we sent from Dubai consulate)
2nd....in system, but only in Manila, not Legazpi...sorry sir, it cannot be
3rd...wife made special trip when heading to Sorsogon....literally just picked it up.

Hi Steph, it's been a busy week for me. :D
Well, I kind of rely on the 2nd sheet on the Manila VO SS regarding who's getting the next PPR.

On your application tho, I would predict maximum of 2 months after you send your PP and other docs.

I heard somebody say that St Lukes asked for a photocopy of their passport page.. so I would recommend keeping one on file.

Our medicals expired almost 2 years ago.. then I checked the GCMS, it said a few things:

Medical - Not Started (latest)
IMM1017 provided are expired
CSCD not entered, wrong visa office on IMM1017 (oldest)

So I guess that means it's okay.. we will need a new one. Hopefully not for the dependents though, it was very tough to get them for 2 days from their un-cooperative Mom. It's not really fair that they opened our application after the medicals expired. If it had been opened within 30 days like it was supposed to be, they would have been still valid.

And hi Iay.. I have had a really busy weekend too. I worked every single day.. today too. The weather in Victoria is unbelievable this weekend.

Does this mean you could get a remedical without waiting for the request? Since it says on the GCMS that you need to start one?

Ohh I didn't know you we're from Victoria :) and yeaa, weather here in Vancity has been the the whole week too. Btw, I'll be in Victoria in May :D my first time. Lol
