I worked in a place where we had contact forms like this. This isn't too far off from what we would get. The people that actually filled out the form correctly would also put their info in the email itself, like so:

Name: Bob LastName
Company: Corporation X
Phone number: xxx-xxx-xxxx, ext 26

Message: My name is Isometry at Corporation X. My question is <question> Please contact me at xxx-xxx-xxxx or through email at isometry @ corpx.com at your earliest convenience.

The people who didn't fill out the forms correctly would also leave the contact information out of their email completely, like so:

Name: Bob
Company: Accountant
Phone number: extension 26

Message: please help

So having the contact fields doesn't help as much as you'd think.

For the age gap, I'd guess that there's no set number, and that what's acceptable varies based on the ages/backgrounds of the applicants. An 18 year old marrying a 30 year old would raise eyebrows, but a 45 year old with a 57 year old wouldn't really attract notice.
 
I definitely agree the ages of both applicants is a factor. A 65 year old marrying a 80 year doesn't seem weird, but a 20 year old marrying a 35 year old will raise some eyebrows.

I did get asked out when I was 29 by a 18 year old. Creeeeeeeeeeeeeeeeepy.

Hey now, you should've taken it as a compliment, haha. When I was 19 I dated a 31 year old for a few months. My mom was very happy when she realized that relationship ended.
 
I already married a younger man/giant child.... not interested for a second round!

I agree. Though, I think it's less about the number and more about the life stages each person is at or their maturity levels. By all means, that doesn't mean there aren't successful couples with big age gaps, but there's a big difference between "I'm excited for frosh week" and "I'm excited for menopause to be over."
