Definitely a big oversight....my heart sank when I remembered.

I can't seem to find a phone number or email address. Would anyone know where I can find it or have it on hand?

When we had a question we emailed our visa office - not that we got an amazing answer, it was basically a copy and paste answer of information we already knew.. but might be a start! Maybe here? http://www.cic.gc.ca/english/information/offices/missions/mexico.asp

I am planning to send mine in soon, just waiting on wifes Police clearance certificate translated in English. Hopefully it will be accepted.
I am just worried that she hasnt changed her last name and added me as a spouse on passport as its from even before we met.

What do you guys think?

Thanks to those who assisted with the contact information to CIC.

Does anyone recall filling out and sending IMM 5620 Payment Fee Form?

I did not send this as it was not in the checklist and I paid my fees online so I included the official receipt as noted. Another girl and myself are wondering if our applications will get sent back because we did not include this form but I still can't find anything that says it was supposed to be sent!

So frustrating! ???

I think this has something to do with the way you paid. When I paid, the receipt it gave me had nothing to fill out so I just through the applicants name on it in pen and sent it. been nearly 2 months and they havent sent mine back. Usually would have by now.

That is what I did too. I still can't find anything on it so I am just gonna try to not drive myself crazy as I would hope if it was necessary that it would have been on the checklist.

Hey it is Kaitlyn from the group!!! I asked in a separate topic and someone states that after September the form is no longer required!!!! Yayyyyy!!!!! And everyone who answered did not send the form and only the receipt!!! We are in the clear!!

I just sent my application outland inside Canada I am currently with my husband as a visitor and my new born living in Alberta and Mississauga received it on January 08th at 11:21am

Oh darn.. well this was to be expected.. we have been contacted by CIC to say we forgot to submit a form! We feel pretty silly.. we just skipped right over it in our million and one checks of the package because we have no dependents. Whoops : it's the IMM5406 Additional Family Information sheet.

Does anyone know.. at the top where my boyfriend has to fill in his information it also says "Spouse or Common-law partner (if applicable)". Does he put my name down? (We are common-law and I am sponsoring him.) Or is that only if he's bringing a common-law with him or something?

At least this means they HAVE our application! ;D

Your husband would put your name in that section regardless. Well that is what we did!
