sakin:

1. doc checklist
2. page 5 ng generic form (with 2 barcodes)
3. page 4 ng generic form (with 8 barcodes)
4. generic form (yung photos na nakalagay sa maliit na plastic ni-clip ko sa generic form. sana ok lang).

pinaka-last yung mailing labels in A4 size.

bale ang wala lang signature and date is yung Travel Form. the rest meron.

Guys, a friend of mine is also applying for FSW 2014, diba dapat maka produce ng COE for the last 10 years of employment? Pero yung company na nagwork siya from 2004-2007 non existent na, ano kaya ang kailangan nyang gawin? Write a letter explaining na dissolved/non-existent na yung company?

That is correct. For the last 10 years. Pero I think yung work experiences lang na relevant sa NOC ang isubmit. Parang may nabasa na ako na yung company nia non-existent na, so ang ginawa nia, gumawa sya ng affidavit containing all the required info from CIC (e.g., JD, length of work, position, annual salary etc) then hinanap nia yung sup/mgr nia sa company na yun ang pinapirmahan nia. Pinalagay nia rin contact number nung sup/mgr. Then pina-notarize sa notary public. I believe it will be fine like this.


Here is a sample format of an affidavit:

I, (Insert Name of Affiant) , Filipino citizen, of legal age, single/married to (Insert Name of Spouse if any), and a resident of (Insert Address of Affiant), after having been duly sworn in accordance with law, hereby depose and say:

He worked full time as a {position} 40 hours/week.

As his supervisor (or any position), he performed the following roles and responsibilities:

1. ...JD...
...
...

2. ...JD...
...
...

2. ...JD...
...
...

This letter has been provided upon request of {your name} for whatever legal purpose it may serve him.

Further Affiant sayeth none.

after carefully reviewing again, so far ang nakita ko na na-miss ko is my postal code in my current address sa Additional Family Information form. Ok lang ba yan? nalimutan ko kasi i-handwritten kasi di nagkasya sa field yung current address ko.

Has anyone also forgot to put at least one postal code in any of you/your family's address sa Additional Family Info? :-[
