You can also send a scanned copy of the new payment form as an attachment and include also you nationality. Good luck, I've seen someone else earlier on this forum who did the same thing and got through. You can refer to page 5577 of this thread for details. I too had to send a new payment form as I had an issue with the bank, thankfully my original payment went through on first attempt so they never had to use the new payment form. My Credit Card has been replaced by a chip credit card due to RBI guidelines. And the card I had used for payment advice to CIC has now been INVALIDATED. My new credit Card has the same number but a different CVV and expiry date.

I am now planning to send a new payment form to CIC with new credit card details. Will be emailing/Couriering/faxing all three documents.
Sending 3 documents now:
1. Cover Letter
2. Payment form
3. Xerox of previous payment form (Cancelled)
In my cover letter I am going to include the following details:
Name:
Passport number:
Date of birth:
NOC Code:
Application courier docket number
Date of receipt of Application by CIC:
Person name to whom package was delivered:
Please let me know if I should include anything else ....?
Any previous cases of similar nature ??? anyone can help me out a bit ??

my application received date:18th July 2014 NOC1112. Cant let my application be rejected....
