in form of account statement or any balance statement at that particular time would be required?

I found the solution. Download a new form but don't tick any check boxes which are not applicable. Because if you tick any unwanted check box from a pair (Eg: YES or NO), then you wont be able to clear the selection. If you wont tick such options, then validation will go through without problems.cian said:-Contact Information-

That's the error I get when I try to validate a form without Fax number.

Okay, so this time, my form was filled with a UCI no. by the VO herself (yea, a lady..her name was Katie ), but the no. is creating confusion. It has got a symbol like 'PHI' in mathematics and a no. 5 is looking like 'S'. blame her for her writing... :-X :-X...so..Can someone tell me how a standard UCI no. looks like so that i can match it with mine and fill it in my new app form...thanx
no worries saini saab, ur name in the passport should match with the BC or any educational certificate carrying ur DOB. so i don't think that would be an issue.
