Hi, We are also based here in SG and when me and my husband tried to get our LOE from our companies in the Philippines they mentioned that they are unable to provide the format we want, so these are the things we did instead:

- We prepared a Letter of Affidavit signed by our previous Supervisor stating all the important details that CIC wants to see and have it notarized;
- We attached the standard Letter of Employment of the company;
- We attached the email of HR from that company stating that they are unable to provide the Letter of Employment from the format we wanted to.

Sa Medical, yes isa lang syang clinic sa Ngee Ann City, bukas ang medical ko balitaan kita. Appointment over the phone lang 260SGD per person. Tapos bigyan ka ng acnowledgment letter for you to upload sa EE for filing. In terms of you knowing the result... not sure eh... pero yung friend namin who did his medical back in July tinawagan sya kasi mataas ang BP nya sa result (high blood ) so sinabihan sya na pababain muna. Nakailang take sya for 3 weeks bago sinabing ok na yung result and isusubmit na nila. So I guess pag may mali naman, iinform nila tayo. Tatanungin ko bukas para sure... sasabihan kita.

Yung Masteral na 1 year part time sa Pinas tapos Bachelor 4y equivalent sa Canada eh may bearing ba? I mean ok lang ba na 1 year tapos part time siya pero may diploma naman tapos Bachelors 4 years ang assessment ng WES?
 
Ano po bah ang document na proof of name change especially sa mga babae na nagchange ang surname after marriage?

Pagkaalam ko po depende po yun sa course and school. Even the same course would have a different equivalency depende if galing sa isang college vs university. Sila lang talaga makakasabi niyan.

Regarding Letter of Affidavit, parang statory declaration ba? Can you please let me know if you only did your own format or does this need to be done by a lawyer? Did you have it notarized sa Pinas or dito sa SG?

This is really a big dillema for me because the company is my current one based here in SG and an affidavit or statutory declaration must be signed in the presence of the notary public, they do not allow any other alternative. Mahigpit sila. Problem is my boss is based in the US and I think it is too much to ask him to have that letter notarized there in the US because it requires his presence. :-\

Ok lang naman na 4y Bachelor yung equivalent, pero iniisip ko kasi sa pag fill up ng EE profile eh tinanong sa education part kung FULL time or PART time yung local degree sa Pinas, iniisip ko na kung may bearing yung PART time at FULL time even though meron diploma na inaward. Baka kasi sabihin nila na kahit may diploma na inaward eh PART time yung local degree so baka i disqualify at need lang nila ay FULL time, may ganun ba?
 
I got this from my relative's medical request from their FSWP application:

Please bring the following to your appointment:

- The attached Medical Report form (IMM1017E)
- Proper identification (proof of identity must include at least one government-issued document
- Eye glasses or contact lenses, if worn
- Any medical reports or test results that you may have as a result of previous or existing medical conditions
- Four recent photographs (photos are not required with photograph and signature, such as a passport or national identity card) enabled” on the CIC website listed above)

I recently did an annual physical exam with my HMO earlier this year, which includes the chest x-ray results (all clear). I also am undergoing biannual tests to monitor a certain condition (which is benign). Has anyone tried to submit previous medical results at their medicals for CIC? I am thinking of doing so at mine.

i'm not sure whether those questions helped, pero with the introduction kasi ng express entry, some of the requirements may have been changed (though ung medical i think is standard)

Federal siya from the FSWP last year, so paper-based, before EE launched. Ako yung representative niya so I got all their communication. I imagine the medicals are exactly the same, dun sa case lang niya may form to present habang tayo sa EE ay upfront so we get the form at the panel physician.
Ako kasi dahil sa case ko, ayoko magka additional test o delays kahit benign which is why I'm considering also sharing my recent results.
Siya kasi pina additional test kasi may finding sa x-ray kaya wala pang PPR.

follow lang po ung assessment ng wes. so if 4yrs bachelors degree ung bingay sayo ng wes then that's all you can claim. you cant claim masters if ndi nman sya na-asses as masters degree. i know some people nag paassess ng bachelors and masters nila pero ang equivalent lang is 2 bachelors degree instead.

My condition isn't clinically significant, so I won't present it, but I can bring the chest x-ray then.
