Hey ImABule, did the person receive DM? I'm pretty sure that they won't send out a DM without letting the applicant explain any red flags in the case.
Well, we didn't file in the representative form and on the checklist we wrote "N/A", but CIC did send my husband the new consent form for GCMS and explained us what to write in each box:

To be valid, a consent must be signed, recently dated, contain the printed name of the person giving consent and identifies you as the designated individual. You may fill the following consent form and put XXXXXX XXXXXX’s information in section A and your information (XXXXXX XXXXXX) in section C:
