I guess, you have to provide the form in any case. Write the e-mail and attach form too along with that. Also, always try to attach a sticky note in such cases when you know the reader can do mistake. This is normal for avaerage people to read i as 1 or 1 as i when both are written together. In future, you can do that.
For now, just send them email with "Change of Information" form.
