Actually, from inside Canada you can order notes for free. If the sponsor is ordering notes for the applicant, he just has to attach the consent form signed by the applicant.

Oh, sorry, I misunderstood your question. I provided my passport at that moment, though my name was written there in both English and native language. For the second one I used either Ontario id or the credit card. But this is just my experience, hope anyone else here can help you with that.
