Oh that is weird indeed, I could always add PDF format of the consent form. And yes within a minute of completion of payment I always got a confirmation email.

You do not have to submit any proof for requesting notes, all that is needed is the consent if you are applying on behalf of the applicant.

Thanks Desijatt. I do not see any comments or notes that indicate any interview yet on last months notes. Hopefully the ADR submitted should suffice and they should start processing the file now..I have requested for another set of notes in the mean time...so the mystery and the anxiety...
