Yes, I applied online with my medicals before I got AOR. Maybe its luck or maybe it's an unwritten rule that CIC don't want us to know about. At the end of the day, just remember that approval depends on the officer handling your file.

As far as I know the medicals covers mainly tests for communicable diseases. you may have to declare your treatment in the background declaration section of your application. All the best!

No, it doesn't have to be in the letterhead. As long as their is a direct contact listed for the person signing the letter you should be fine
