Is there a Canadian embassy near you reside? I think the best is to either call or visit the nearest Canadian embassy and give them your client ID (client ID for PGWP is the same as your number indicated on your study permit). hope it works out for you ;)

oh wait, spoke too soon... I don't think you are allowed to be registered in school if you are on PGWP. It may not apply to you as you are studying in the states...

You can just do it on-line and you need to pay another feel. I used the same application as PGWP and stated the reasons for extension. I sent the copy of new passport, letter from my employer, and copy of my degree.

I am planning on sending the application via FedEx. When sending by FedEx, they'll give you their envelop and a sheet to fill in the address. So it won't look like the one provided in the instruction, like how they want your name on the top left hand side and their address in the middle...
