You can directly apply to provinces if you meet the criteria. Some already introduced a new stream called Express Entry (e.g BC) and some are still working on it(e.g Ontario).

Wrong! CIC does not recognize working hours more than 30hours/week. In other word, you cannot compensate weeks worked less than 30hours with other weeks that exceed 30hours.
BTW , I think you will be OK since you got paid for the period.

Yes. Actually there is a minimum 67 out of 100 points requirement for FSW stream. Check CIC website and do the math to see if you need relative points to pass the criteria. I suggest you to calculate both scenarios using FSW system and in case both passed the requirements, go for the one having...
As far as I am concerned, you mus also meet minimum Federal requirements to get a nomination from Provinces under EE unless you take a regular path.
BTW, it's not fully clarified yet and I might be wrong.

c) LMIA will give candidates extra 600 points but it's not mandatory
