What did you submit to address the reasons?
Family Ties: name and address of all family members. only my(sponsor's) address is from Canada. My wife's family and my family is in India. Earlier I thought family info form is sufficient but in second attempt I included my wife's family info plus my family info in complete detail in cover letter of my wife similar to family info form format.
Funds: Earlier I wrote financial support letter for my wife because my wife didn't have plenty of money in her account. This time her father put some money in her account and gave a notarized letter verifying the transaction and purpose of transaction. Now my wife has sufficient money in her account which she directly have access to. Also financial support from me. (air tickets, health insurance, hotel bookings etc. fully paid by me)
Travel history: My wife has no travel history but in cover letter showed her interest in traveling in future along with me(husband) to different locations other than Canada as well. But since she has support and ties in Canada so we showed her interest of visiting Canada as a first destination.
other than these points, all requirements were meeting already like our employment status, relationship proofs, medical, PCC, Tax forms, immigration status etc. I guess it's imp to include all details in your cover letter and attach documents which support your statements and whole story.
hope this gives some idea to other applicants.
