In Vienna's defense. This money goes to the Medical Practitioner, not to the Visa Office.
 
We flagpoled this morning at the Prescott border crossing. It was really quick and hubby is officially a PR! Finally! :)

I know...I think the medical is already expensive for the few minutes you are in there, so it really sucks for people that have to redo the exam. I hope I don't have to redo it.
They forgot to tell me that I had to bring 6 passport pictures (instead of 4) because I'm applying outland. So I had to go back in later to give an extra 2 pictures, and the dr. charged me an extra 40 dollars. However, it will be all worth it if the process gets finalized.

Where do I have to do the medical tests?
I am in Canada with a tourist visa!

And is it always that you have to do an interview?
And where in Canada or in Germany?
I am from Germany!

In the application process time ...can I work do I get a work permit?
If I am in Canada!

You can do your medical in Canada. The results will be forwarded to your visa office (Vienna in your case).
Not everyone has to do an interview, it's on a case per case basis, depends on the proof you submit, and your relationship history etc. If they do call you for an interview, you have to go to Vienna.
If you're applying outland (Vienna) you cannot apply for a work permit at the same time (this is only for inland route). Did you look into working holiday visa if you're under 35? That would allow you to get an open work permit for a year.

My spouse had her medicals done on January 31st 2013 in Skopje FyroMacedonia. The process was very professional and the cost was only 120 euros (approx 7000 denars)
We had to bring only 4 photos..I wonder why they asked you for 6? Ours is also outland!
And why did they charge you an extra 40 dollars. Just so they can file the extra 2 photos? This is ridiculous!
