CIC's criteria is to attempt to assess whether the child would require any significant assistance going through his life in Canada.
They have a practical measure of assessing if he would need more than 6k CAD per annum in health care supported by government once he lives here or not. There is likely a refusal if the cost is significantly higher.
To give you an estimate, kids in Canada receives speech therapy for an hour a week in schools if they need it. And cost ranges between 100 to 150 CAD an hour. Government insurance or even private one might support like 600 to 1k per year of speech therapy. Rest one pays himself.
