That white tape you see is supposed to be peeled off and an original photo stickied onto it by them, then the photo stamped. Did you not have enough photos? One has to go on that, one on their copy, one on the xray...etc. You have to send in the original (photocopy one for yourself for your records), WITH photo on it so they have something to compare the one at the place they send the forms to.luckcy said:I am in USA. The IMM1017 which DMP gave me (green) doesn't have my photo on it. In the place of the photo there is a white tape with a stamp of a maple leaf and the number. The DMP did sign his name on it. But he didn't ask me to sign. Is that right?
At least I know this is how the outland process goes for those doing meds in Canada to file in the USA. If it's any other way in the US, someone correct me, and I'm sorry.
