Just goes to show, it's better safe than sorry to include it even though you're also including proof you are actually a couple in your application. What CIC really needs to do is to put that form in both Part 1 and Part 2 of the list of forms here, just like it does with Use of a Representative: http://www.cic.gc.ca/english/information/applications/fc.asp. And yep, you send it to the HSBC Center address.

They need my statutory declaration of common law form. In the checklist it states this is not necessary unless I have a common law partner co-sponsoring. I now have to schedule a public notary to watch us sign this paper regardless of the stack of information I sent with my application. What a pain. Hopefully this doesn't slow down my application.
Do I send this back to the address on the bottom of the paper?
