The notes you are referring to say that the officer is not satisfied with the work reference, and the applicant stated that he submitted letters not signed by hr, and you join 2 and 2 to make it 4, whereas the issue could be anything ranging from NOC job duties, employer verification, issues in dates, salary etc.
Submitting documents that are in stark contrast to the ones submitted earlier, will only make issue worse.
Before just throwing something, make sure you understand the repercussions.
