Is it OK if I will use those forms and send it to them? I mean, filling them up online, print them and send it to them. Got problems with my penmanship, that's why
I have completed filling the forms, just have to notarized my documents and its ready to go by tomorrowjpadz said:Thanks Moiz!

In your wife's copy, did she put her name on the APPLICANT section?cchabert said:I fill the IMM 5406 with all my family and then made another one with all the info from my wife
You have to fill two separate forms. One for you and another for your spouse (if applicable). In your form you list your family details and in your spouse forms she will list her family details.jpadz said:To Senior Members:
On how to fill-up ADDITIONAL FAMILY INFO section of IMM 5406:
1. Do the family members pertain to the principal applicant?
- like mother, father, brothers and sisters
2. Does the accompanying spouse need to fill-up a separate copy of it?
Please help. Thanks!
