mga ka forum mates naghahanap ako ng IMM 1017 which is para sa medical wala akong mahanap yun pala wala tlga.. so pano pagpunta ko sa saint lukes sasabihin ko lng papamedical ako for canada visa ganun ba? tpos alam na nila yon. salamat..

Wala kang dadalhing form kasi dun ka na ffill out ng form. Basta family class wala dala form. Tas requirements lang dala mo ganun.

Hello po. Patulong naman po, sa mga married po dito sa Canadian National, ano po nilagay nyo na middle name ng asawa nyo sa Marriage Certificate? Nung nagapply po kasi kami for marriage license, I was told na ang ilagay daw po namin pareho eh yung maiden surname ng mother namin. So pati yung asawa ko, kahit aware kami na hindi naman uso sa kanila yun, nilagay nya maiden name ng mother nya. Magkakaproblema po ba kami sa application namin?

hello, in my case, Hindi na namin nilagay yung mother's maiden name as middle name niya kasi wala naman daw siyang any document na nagsasabing middle name niya yun and besides, hindi uso sa kanila yun. so far, wala namang naging problem (thank God)...tinanggap sa lcr and nso...

Hi Sis, I'm March 2014 applicant. My husband called CIC to inquire on when our file was transferred to CEM, it's okay to inquire sis or email them, they will answer you within 24 to 48 hours, kasi nag email din ako last week.

hi! it means electronic client application status. it's where you check what's going on with your file using whatever the CIC or CEM gave you or your sponsor (e.g. UCI, File Number etc.)

RE-ASSESSMENT OF A MEDICAL FILE FOR A NEW MEDICAL CERTIFICATE
Dear Applicant:
In order to have a re-assessment of your medical, we will require the following:
1. Has the health status of this applicant changed since the last immigration medical exam?
2. Has the client been in close contact with someone with active pulmonary tuberculosis since the last exam?
3. CHEST X-RAY (film/ CD and report)
NOTE TO THE PANEL PHYSICIAN: Please ensure that the first two questions are answered.
Please return to the Panel Physician who performed your immigration medical examination or to another eMedical enabled Panel Physician (http://www.cic.gc.ca/dmp-md/medical.aspx). The Panel Physician will arrange for the additional medical tests to be carried out and will forward the results directly to this office. The PP must ensure that this letter and the results are sent to this office via eMEDICAL.
Once we have completed our medical re-assessment we will transmit our decision to the appropriate Immigration Office.
If the additional medical information is not received within 30 calendar days, your file will be assessed based on the information available, which may result in a refusal of your application.
