I'm still using my old names yun kasi ang nasa passport ko and other valid id's . first- Under GENERIC APPLICATION FORM FOR CANADA about personal details Q#2.a)have you ever used other name(e.g. nickname, maiden name,alias,etc?)b)If you answered ''yes''to question 2a),pls provide the name (e.g. nickname,maiden name ,alias,etc.) Since old name parin ang gamit ko , pwedi ba itong maiden name ko din? second- under ADDITIONAL DEPENDANTS/DECLARATION FORM , para sa applicant ba ito na form? Q#11 a)Relationship to principal applicant? EDUCATION/OCCOPATION DETAIL 4 Intended occupation, ano po ba ibig sabihin nito? The forms are always asking for GIVEN NAMES AND FAMILY NAMES what about our middle name or surname?

Yes true, confusing naman talaga yung mga questions.

sabi mo yung surname mo pa ang use, di pa kayo kasal? Mas maganda kase if yung married surname na ang gamit mo pero meron din iba na surname pa rin nila ang gamit later na lang nagpa change di ko lang alam pano. Q#2. have you ever used other name(nickname, maiden name, alias etc), I am not sure sis eh, pero pwede naman siguro yan maiden name mo.

second- under ADDITIONAL DEPENDANTS/DECLARATION FORM , para sa applicant ba ito na form? para sa dependent/anak mo yan kung meron anak, kung ikaw lang no need to fill up that, sa pgkaka alala ko pa pag nag fill ka na sa computer mare remove yang form na Additional dependant, don’t forget yung barcode page importante yun!

EDUCATION/OCCOPATION DETAIL 4 Intended occupation, ano po ba ibig sabihin nito? Sagot ko dyan housewife.

Family Name/Surname/Last Name: Serrano
Given Name: Rachel Angel Cruz

Ganyan ang fill up na ginawa ko sis, yung middle name na Cruz sinasama ko sa given name para buo. Nagtatalo rin kami nuon hubby na yung middle name ko daw eh yung second name ko, kako dalawa nga pangalan ko pano magiging middle name ko yun, diba ang middle name natin yung apelyido nang ating ina!


Your welcome sis, ganyan din ako nuon dami tanong, yes as in lahat nang proofs remittance receipts, pictures, cards, letters, chats, air tickets nung tour sa boracay or somewhere at kung anu-ano pa… Alam ko gusto nyo na matapos ang mg fill up para maipasa mo sa sponsor mo at maipasa na nya sa Mississauga for sponsor approval at maforward dito embassy, siguro nagfi fill up na rin sponsor mo. ;D

Lynnie24- there must be a way for them to get another original copy from where their divorce was granted. I dom't really know how it goes but, I will ask my husband later and I will give you an update.


rainjohhnny, so if ever they will request for the original one of the divorce certificate my husband then need to get it and send it back to me?? (if they will request for the original one)...i just spoke with my hubby about this and he said that we will just wait what documents they are going to ask from us...coz we just don't know if the same ba ang mga documents na hinihingi nila sa mga applicants.....well see and will just keep in touch... thanks.

True. Lynnie24. It's a case to case basis. If you can't comply/ give them the original divorce papers you can have a photocopy of it notarized naman daw.

Oh ok. stay patient kc waiting na naman pagnagremed. I got mine last April 21 kaya super stress na kakahintay. I heard roughly 3 months to get the visa so wait wait wait. Good luck and God bless!

I dont see the point why they keep on asking for the passports yet they arent issuing visas. Frustrating indeed!



Hi dumas89, when they request for our passport the next thing will be visa will be attached to our passport when they returned our passport back to us....cg lng darating din ang visa na hinihintay natin for sure....we will have it in Gods own due time... God Bless!
