Good Day Everyone.. May i ask what does Given Name(s) in most of the forms mean.

Does this mean only the name, or name with the maiden name.

Family Name(s) = would be husband surname
Given Names(s) = would be?

My Passport Show the following

Given Name : Actual Name
Middle Name: Maiden Surname
Last Name : Husband Surname

Yung COPR ko kse dati pinagsama ng CEM yung Actual Name and Middle Name sa Given Name - I think they based it dun sa info sa PH Passport... eh since yung PH passport has 3 names and yung COPR doc has only Given Name and Family Name... kaya pinagsasama nila yung Actual Name and Middle Name sa Given Name.... I hope hindi kita na-confused more... ;D

Bigyan Kita ng sample sis to better help kasi iba dito sa Canada hindi katulad sa pinas

Sample name: Maria Kristina Santos Gonzalez

Pinas:
First Name: Maria
Middle Name: Santos
Surname: Gonzalez

Canada:
First Name: Maria
Middle Name: Kristina
Surname: Gonzalez

Mas preferred yung una kong sinabi kasi I ran into this problem kasi may Maria ako in front of my actual name so I have to write Maria as my first name even though yung Second name ko talaga ang ginagamit ko and not Maria.

Ung sa akin ang nilagay ko lang is my given name hindi ko sinama ang middle nme kc yn lng nmn ang tinatanong. Sa application form ng sponsorhip n finill upan natin nakaseparate ang middle nme.

I checked my extends visit permit docs from Cic, hindi n nila sinama ung middle name ko. Just sharing what i did and what i have.

Question po, Gusto ko po kase umorder nung GCMS notes para mkita ko po kung ano n ngyayare sa papers husband ko, Ako po yung sponsor, and alam ko po kelngan po ng consent kase information po yun ni husband, but kelngan po ng signature po. Pwede po b na pag nasignan n nia yung consent, pwede po b n scan nia nlng then email nia saken o it has to be the original copy po n kelngan nia pa isend saken by mail? Confuse po ako.. Thank You!

Your husband can sign it and then scan and send it to you sis. Dont forget to sign ur part. You have then scan it and attach the consent when u order ur notes.
