My wife (residing in the Philippines) and I got an email last month from the embassy. They were asking for:
• Fully Completed Appendix A (attached) for you and each of your family members, whether
accompanying you to Canada or not. Passport details are not required for any family member
who is not accompanying you to Canada. (Each person should complete his/her own form.)
• Valid National Bureau of Investigation (NBI) issued under the name: ***** (travelling to Canada), issued within the last six months, with
thumbprint and dry seal affixed. Please send us the original copy. “Personal Copy” is not
accepted. DO NOT FAX YOUR NBI CLEARANCE.
• Original NSO Advisory on Marriages for yourself (see sample)
• Original Passport(s) for you and each family member who will accompany you to Canada
Questions:
1. We're confused about the Appendix A. We both do not have children or have children together yet. She's the only one I'm sponsoring.
We don't need to fill this out correct? Do I need to fill out one?
My initial assumption is that we're supposed to put her parents and siblings names in that form.
2. How and where to send these documents?

1. We submitted our ppwk late February
2. Notification that they received the ppwk in April
3. Request for new ppwk last August
I hope that's a good sign. I wish to be reunited with her around April-May of next year.
