accompanying you to Canada or not. Passport details are not required for any family member

• Original NSO Advisory on Marriages for yourself (see sample)
• Original Passport(s) for you and each family member who will accompany you to Canada
Questions:
1. We're confused about the Appendix A. We both do not have children or have children together yet. She's the only one I'm sponsoring.
We don't need to fill this out correct? Do I need to fill out one?
My initial assumption is that we're supposed to put her parents and siblings names in that form.
2. How and where to send these documents?

1. We submitted our ppwk late February
2. Notification that they received the ppwk in April
3. Request for new ppwk last August
I hope that's a good sign. I wish to be reunited with her around April-May of next year.
I got the same email from them last Aug 2013. You need to fill out and give whatever they ask for, if you miss even one form, it will be incomplete and you may have to wait longer for the visa
Appendix A - you need to fill out one form only. Just for your wife. Cause you mentioned you don't have children, leave the child 1-5 boxes blank, make sure fill it out, and sign it
NBI - it should be issued within 6 months. If you need to get one it will be pretty fast to get it, if your do not have the same name as someone, if your name is common, then it may take a few hours or days. My husband has a really unique name, it took him less than 5 mins to get.
Original NSO - look at the sample they attached
Passport - send your wife's passport
For the address. It says "Visa Section" on the top. I don't have exact office on me. but it is in the email they asked you to send the additional doc. plus passport
