I have a questions regarding police check.
I have been living in canada for almost 5 years as a student, now on a work permit. When I first applied for my study permit in Italy, I provided a police record check from my country. I am wondering if I need to submit a criminal record check from Canada or my own country or both?
Has anyone been in the same situation before? What did you do?
Also How long is the medical valid for? We want to send everything altogether so Im thinking to get the medical done before they request it.

You have to submit a police report from every country you have lived for more than 6 months after the age of 18. In your case, that seem to be Italy and Canada.

Medicals are valid for a year. I would recommend making an appointment near the time you expect to have the police reports (or all the docs gathered) for the medical to be as new as possible by the time you submit your application so the chances of having another medical requested are reduced; medicals are to be sent with the application; I have read about cases where medicals are missing and the application is returned or the application is put on hold until medicals are received.

You don't need to get a police check in advance from Canada to include in your application. This is done by CIC during the processing.

Rob_TO is correct. CIC may ask you to get the Police Certificate from Canada done later on during the processing of your applications.

In reading lots of posts on this site, i've never come across one where a sponsor or applicant has actually had to get a Canadian police check done independently and send to CIC (though perhaps it does happen occasionally).

It seems like I don't need to send a police check as I left back home weeks after turning 18. And like you said they can always check with RCMP themselves.

I have read some posts where CIC has requested from the applicant to contact RCMP and what I gather from the process is that RCMP may send the report directly to CIC or VO; at least that is what I assume by reading this statement from the RCMP website:

** Applicants for immigration to Canada must enter the name and address of the Canadian Immigration Center, Embassy, Consulate or High Commission handling the case in the "Name and Address of Contributing Agency/Dept" portion of the C216C.

Maybe someone that has done it in the recent past may have an input.

Note: unless you are explicitly asked, you do not need to provide a police report for Canada. CIC will query RCMP directly, they don't need you to do it. The only time they ask you to do so is if they need a fingerprint search which happens if there is a match against your biographical data (e.g., someone with your name has a criminal record).
 
when you apply for pr do you have to apply for an open work permit as well or can you wait for immigration to finish the application

It's best to apply all at once including paying all the fees at once. It makes the OWP faster because you don't have to wait for the approval, then mail in the application and wait for the processing all over again. I've also heard of stories where the couple sent in the OWP after getting AIP and CIC couldn't figure out it went with a PR application, so they rejected it. Genius!
