i'm being sponsored by my husband and got PPR already. today, i got a medical surveillance form and i know this means i have to report to a health clinic for monitoring. no further message, just the forms.

can anyone enlighten us on the process? how long is the surveillance normally? can this be completed in a week only?
i plan to stay in Canada for a short period only (1/2 month only) and go back to Philippines to continue my work and studies.
should i declare this in immigration and to the health office as a change in address? will i be able to complete my health check or is it just a one-time reporting to health office?

RPRF is only $490

  • dependent children of a principal applicant or sponsor, a child to be adopted, or an orphaned brother, sister, niece, nephew or grandchild; and
  • protected persons, including Convention refugees.

If you declare your plans of returning to the Philippines right away , CBSA officer may cancel your CoPR and advise you to re-apply when you are ready to fully move to Canada .
 
Dear Members,
I need your help. In generic application form i forgot to write down my email address and i have already validated the form, signed it and sent it to my sponsor in Canada. Now advice me what could be done?? Can I put my email and print another version of page 1? or I should re fill the form, validate it again but use the same previous last page as it is signed by me. Please help!!!!

All pages of the form must be from the same validation code. I've tried this because I committed errors as well, and upon printing, I've noticed that the series of numbers changed when validated. Thus prompting me to just do it all over again to avoid discrepancy.

If you still can afford to re-do the form and send it to your spouse, then that's the best thing you can do.

Is it ok to write it down via pen on printed page? some one said to me its ok if you missed your email, your email is already mentioned and it is enough to have them one email address only.

You know the reason you missed it because it doesn't have an asterisk, meaning it can be optional to put it, although it is highly encouraged to have it on the form.

Do not write anything yet as we are not sure if that is okay.

I have another idea if I could write down my email on another page explaining my situation, sign it and send it to my sponsor so he could attach it with the application.

Because this can be scanned. I don't know If its ok to use the scanned and printed forms for our application as I have heard only originals are acceptable.

You cannot use pen on a validated form. Best option is to re-do it and send it to your sponsor in order to avoid delays.
 
i have a question regarding my application. cic send me a mail for a week now because one of the application doesnt have enough info but i havent recieve any mail regarding that what should i do?

I will be submitting family sponsorship application for my wife this week.

As a sponsor, I wanted to confirm about documents which we need to submit. Following are documents i am attaching with sponsorship forms:

1. PR card photocopy
2. Notice of assessment and T4A
3. Proof of employment.

Do I need to submit more documents for sponsorship approval?

Add your Driving License copy as well buddy. That shows where you are staying.

Did they mention which form at least ? If not ask through IRCC Webform and they will let you know. If you know the form number then go through the website which says how you fill the form and check if you have missed anything. Most likely the dates and signature.
