I received remed request today where else the medical results on the status shows passed and it was done on april 27th 2015, how do I handle this situation ?

there are couple of applicants who received remed request while their med was not expired can one of them guide me thru this

If you did it in April 2015 so it has already expired. They are now updating ur file to process ur application. Just follow the instructions of the letter and do the remedical

if our CNIC is renewed while application is in process do v need to inform VO, do we need to get it stamped from notary public? also if it's a SMART CNIC (meaning in English and Urdu both) do v need to get it translated since the address is only in Urdu?

Alhumdulilah our case is done. We received PPR request on December 19th got passport and COPR on December 27th.

The change in medical review is obviously because they have requested remedical. Not sure why they would ask again : but we have seen them do that over here with few members. Just do the re-medical inshallah should be fine.
