Hi Micky,

I guess I'm the one you are talking about. I did apply in Mar-2014 and couple of weeks ago I got the email from LVO to submit color copy of passport bio page along with latest photographs which I have dispatched last week and now waiting for their further feedback. I'm assuming that this is PPR1 as I'm early 2014 applicant and at that time passport request was two steps process. 1- PPR1 - Copy of passport along with current photograph suppose to send via courier to LVO. 2- PPR2 - Original pass port suppose to send to Islamabad for visa and COPR.

ALLAH PAK agay ka kam bhi asan farmain. Aamin

They only asked me to submit a copy of passport having information page only . they not asked me for color copy or photographs

It seems ISB is taking a long time we submitted ours on the 11 of nov.
Still have not received pp. back
Why are they taking so long???

thanks, I think it takes 2-3 weeks for DM after re-medial. After one week, I did sent csi and an email to LVO but didn't get any reply.

So LVO had FORGOTTEN to tell us to do the re-medical. And so after I got my notes I noticed that the medical had expired 3 months ago and they said they sent us an email. I sent them an email asking what happened etc. and they realized and finally sent us re-medical.

Completed MEDICAL REPORT CHEST X-RAY REQUISITION AND REPORT form (Page 5 and 6 of IMM5419) (in PDF)
Duly completed Re-assessment Form (in PDF/word)

is there a website I can find all 3 of these forms?
Also is my husband suppose to print these 3 documents and give it to the panel physican as a check list?

we got the exact same e-mail from RMO last week. Tell hubby to forward this e-mail to IOM and get them to make him an appointment. He only needs to print this e-mail, and print the form that is attached to it, and take it along to the IOM when he goes...

When they e-mail you the appointment date and time they will tell him what to bring and how much it will charge, and also that there is no parking at that place (so inconvenient IMO)...

CANMEDPK@iom.int is the e-mail address. DO this right now so that when the office opens they give you an appointment right away!
