these questions for IMM0008 form

Should the main applicant and the accompanying spouse fill the form, or one form for the whole family?

In the IMM008 form, National Identity Document section, they ask for an expiry date, but in my country ID has no expiry date, and i cant leave it blank or write anything but a date, what's your advise for this?

My daughter is less than 6 months old, and they need a number of years of education? can i write 0?

, i have never been in Quebec before, and there is no space that i can type N/A, its just tables of dates, what do you advise me to do ??

I am planning to apply for QSW. I have started organising all the relevant documents except TEF. Currently I'm enrolling in an intensive French Class and plan to take TEFaq by January or February 2012. right after I get TEFaq certificate, I'll send my application for immigration process. By late March 2012, I'l be 37 years old. My question is: if i send my application by February or early march 2012, can I claim 14 points (CMIIW) for my age (36 yrs old), or I only can get 12 points becase the AOR is April or Mei 2012. So what is the cut off for my age point? is it AOR?

and what about the spaces in question 18?? should these filled with N/A??
