I applied for the FSW 2014 program and have successfully received the PR visa. Below is my timeline:

PER: 30-Jan-2015
AOR: 24-Feb-2015

Med Done: 11-May-2015

Med's Received: 14-Oct-2015
Passport Request: 22-Dec-2015
Passport Submitted: 24-Dec-215
Passport Received with Maple leaf: 30-Dec-2015
Since mine was a furtherance medical case, I received a "Medical Surveillance Undertaking Form" attached in the PPR email along with the following line:
When you enter Canada, you will need to provide two (2) copies of this form at the Canadian Port of Entry together with passport and Confirmation of Permanent Residence
However I did not receive any separate surveillance form along with my COPR from the embassy when I submitted my passports for stamping.
My concern is:
Is the "Medical Surveillance Undertaking Form" received with PPR email suffice or should I have received any separate form from the embassy along with COPR? Anyone who has faced such a scenario can please reply if they were provided a separate surveillance form to accompany the COPR?
